Reablement
What is Reablement?
Reablement is a free service for South Ayrshire residents aged 18+ that provides tailored support to people in their own homes for a 6 week assessment period. The service aims to assist people to remain as independent as possible by supporting them to regain daily living skills and confidence after a hospital admission, a period of illness or an accident. We will work with you and your family/carers to identify your goals and key tasks that are important to you in your day to day life, supporting you to regain skills to help you to achieve your goals and complete these tasks on your own, within agreed timescales.
How Reablement Works
Reablement starts with an assessment (a discussion of your needs with you and your family/carers) of what you can currently do for yourself, what you used to be able to do prior to your hospital admission, illness or accident and sets goals for what you would like to be able to do. This is carried out in your own home by an Occupational Therapist or Occupational Therapy Assistant from the Reablement team who will agree a support plan with you to work towards your independence.
We work with you on the areas identified in your support plan to help you regain independence and confidence. The amount of support you receive during and after your period of Reablement will depend on your needs and progress, and is likely to change accordingly.
We will review and monitor your progress regularly and update the support plan as your levels of independence improve. A copy of your support plan will be kept in your home so that you, your family and carers can keep track of your progress and what your goals are.
We will also identify and arrange any appropriate equipment or onward referrals. A final review (discussion) will be carried out at the end of the Reablement period with your progress considered. If you require ongoing care this will be agreed and organised with you.

Referral Routes
- Community referrals: If you feel that yourself or a relative is in need of support, contact 0300 123 0900 to request an assessment.
- Hospital referrals: If you or a relative are in hospital, speak to a member of ward staff who will contact the hospital team.
