Tier 4 Specialised Care

Specialist Integrated Care

For people with complex, high-risk needs who need round-the-clock, proactive support at home.

What makes Tier 4 different?

Tier 4 could combine smart home sensors, predictive data analytics and a 24/7 responder team with hands-on nursing and social-care input. Every package is built around one person's risks and goals, likely no two plans are the same. South Ayrshire have an Intermediary Care Team and Hospital at Home in operation

Who is Tier 4 for?

  • people with frequent falls, brittle longterm conditions or advanced dementia
  • those recently discharged after repeat hospital admissions
  • carers who cannot stay overnight but need absolute peace of mind

A professional (GP, district nurse, social worker or hospital team) must confirm Tier 4 is clinically and socially appropriate.

Responder team & NHS Out-of-Hours

Monthly reviews

Data trends + human feedback = tweak sensors, meds or rehab goals

Whole MDT
