What makes Tier 4 different?
Tier 4 could combine smart home sensors, predictive data analytics and a 24/7 responder team with hands-on nursing and social-care input. Every package is built around one person's risks and goals, likely no two plans are the same. South Ayrshire have an Intermediary Care Team and Hospital at Home in operation
Who is Tier 4 for?
- people with frequent falls, brittle longterm conditions or advanced dementia
- those recently discharged after repeat hospital admissions
- carers who cannot stay overnight but need absolute peace of mind
A professional (GP, district nurse, social worker or hospital team) must confirm Tier 4 is clinically and socially appropriate.

Costs & funding (TBC)
Tier 4 technology and responder visits are funded as part of your social-work care package or NHS Continuing Care; no weekly client fee. Any changes are agreed at your review meeting. (If self-funding, we'll give a written quote first.)

Data are stored on an encrypted, approved cloud. Only professionals involved in your care can see your information. We never sell or pass data to third parties.
