How can you be referred?

Who can refer?

A GP or GP surgery attached Mental Health Practitioner working within the GP practice can refer you to the team.

 

Referrals can also be made by other NHS Services such as:

  • Mental health in-patient services.
  • Mental Health Unscheduled care teams
  • Mental Health Liaison Services (based within General Hospitals)
  • Drug and Alcohol Services
  • Other Partner Agencies

If you've used our service before and feel you need support again, you may be able to self-refer through a re-engagement plan. This means you can get back in touch with us directly, without needing a new referral from your GP or another professional.
