Practice Social Work Teams

Our Practice Social Work Teams give longer-term support to adults with more complex needs. They create care plans using Self-Directed Support (SDS), check if 24-hour care is needed, and help people live safely and independently.

They also assess carers (where eligible) and oversee Welfare Guardianship orders. If someone needs ongoing help, a Practice Team Social Worker is assigned to manage their care plan. After the plan is in place, it's reviewed every year unless changes are needed sooner.
